Please read the statements below, confirming you understand the following completely:
- That no warranty or guarantee has been made to me as a result of this hair extensions procedure, and that the final result cannot be guaranteed.
- I will be charged for a Blowout which includes installation of halo hair extensions, plus the halo hair extensions fee of $150 for 1 day rental. Total of $200 at time of service.
- Each additional day is $50 - which will apply based on time of service. If you require an additional day, please contact a spa representative to inform them of expected return day and time in case required for other clients that are booked.
- An authorization of $500 will be on account until return of hair extensions. If hair extensions are not returned by due date this amount will be charged on to your credit card provided. This includes if the Hidden Crown halo hair extensions was lost, stolen or damaged beyond repair.
- I understand and acknowledge that I am fully responsible for returning the Hidden Crown Halo hair extensions in the same condition I was given it.
- DO NOT WASH or use any hair products in halo hair extensions. I acknowledge that I am NOT responsible for washing the Hidden Crown halo hair extensions before returning to Glow Day Spa. It is included in the price of rental.
- DO NOT wear the halo hair extensions in a smoke environment (nightclub, bonfire, cigarette, cigars etc.) - the halo hair extensions needs to be returned "smoke-free" or an additional cleaning fee will apply and deducted from authorization.
- I will carefully take out the halo hair extensions and use provided storage bag and hanger for hair to be returned in original condition.
- While using the halo hair extensions: do not take a shower (getting hair wet); avoid any saunas or jacuzzis; avoid tanning beds which may change the colour of the hair.
- I understand and will follow the aftercare and return instructions and will use advised storage after the hair extensions are not being used or taken out of hair.
- I understand to follow instructions from the stylist throughout the hair extensions service as damage could occur if not followed instructions.
- I understand if there is any damage or unauthorized changes made to the Hidden Crown halo hair extensions, I am responsible for purchasing the Hidden Crown halo hair extensions at it's original cost. estimated $500 or more.
- I hereby release any and all persons representing Glow Day Spa Inc. from all claims, demands, damages, actions and cause of action arising out of the result of the service.
- I agree and understand that I am not permitted to altering the hair extensions colour, length (cutting) and wiring of halo on the Hidden Crown Halo hair extensions.
The undersigned hereby consent to Glow Day Spa Inc. taking photographs of the undersigned both before and after the extension installation by the stylist at Glow Day Spa at the request of undersigned.
It is further understood, acknowledged and herein authorized for Glow Day Spa to use said photographs for compiling an album of its various clients for showing prospective clients the procedures completed by Glow Day Spa in its normal course of business. If in the event the undersigned does not wish Glow Day Spa to use said photographs in its prospective client photo album, then the undersigned shall expressly to state by completing the non-consent form (for marketing) provided and the undersigned there after acknowledges and consents to Glow Day Spa retaining all photographs solely for its own files for its own use internally in the development and monitoring all services provided.
I certify that I completely understand and comply with the above as stated:
